Prom Perfect Drive Request Form

We are excited to announce our first-ever Prom Perfect Drive! We recognize how important prom is—and we want to help make it a joyful, unforgettable experience for the youth we serve.As part of this initiative, we invite programs to submit requests for prom attire and accessories.

Please note that this is a donation-based drive, meaning all items have been generously donated by community members. While we will do our utmost to fulfill as many requests as possible, availability depends entirely on the donations we receive, and we cannot guarantee that every request will be fully met.

Requests will be processed on a first-come, first-served basis.

All requests must be submitted by April 30th—no exceptions. If you have any questions, please email Toni-Ann Campbell (tocampbell@childrensvillage.org).
